    In Intensive treatment and then in Secure transit (elevator shaft) is the riddle "Dr. Jonathan Crane plans on elevating fear to new depths"

    Look under the elevators ;)

    haha,,, well thank you for the hint instead of telling me straight out. I beat the game and was 239 of 240 secrets without help and was exhausted and couldn't figure it out and gave up....woke up today and solved it in 2 minutes....man i know that elevator shaft now better than the guy who designed it. 
     
    props to rock steady for making colletables that you can actually find......240, i never thought i'd get them all.
