You gotta beat the game. It then starts after that.

Only if you don't die does the game carry on with Broken Steel.  If you got the Supermutant companion then you can use him to activate the purifier and he won't die cause he's immune to radiation.

Is this true? Don't you awaken from a coma regardless in Broken Steel?

    Haha. Whoops.

If it makes you feel any better--which it probably won't--the ending itself isn't that impressive. What is impressive is what the Brotherhood gives you to help wipe out the Enclave in the last bit of the game.

Actually i don't think i've tried it but that's how i assume it works.  You didn't have the option to use Fawkes in the original ending so i assumed they added that in as an alternative to the "self-sacrifice" one.

No, it actually doesn't work. You are never given the option to ask Fawkes to do it. Kind of a plot hole.

Just pretend Broken Steel isn't there and beat the game normally. No matter what you do, you'll live.

Doesn't matter HOW you end the game, Broken Steel will start up afterwards.

@dude2242: Make sure it's installed (check out the memory/games section of Xbox's HDD). I'm going to guess it isn't there, or it's corrupt - either way, you'll have to find a way to reinstall it. This would mean getting a new GOTY edition, or buying Broken Steel from the Marketplace.

Make sure you play long enough for it to say "Broken Steel loaded. Level cap raised to 30"

If you do it before then the game will end per usual.

When I goto "downloads" off the main screen, should Broken Steel show up? I even deleted the DLC and re dl'd and installed it. Still nothing.
