The funniest & worst glitch I've run into was I got stuck in VATS yesterday while escorting Red from Germantown when I attacked a Super Mutant. It was lame but hilarious (i totally saved right b4)
I really haven't had any killer technical errors/glitches since I started playin, any1 else?

I was in a supermarket and had killed all the raiders in the entire building and gone into the backroom, lockpicked all sorts of stuff and scored some schwag, then as I was a bout to leave the room I accidentally closed the door. Then I opened it up again, but for some reason every time I tried exiting the open door, I was thrust back into the room. I had to reload from my last autosave and kill all the raiders over again.
Other than that I didn't run into any huge glitches.

I was playing the game, and it froze my Xbox. It then red-ringed. When I put it back in after repairs, I loaded my save file (all but 1 got deleted) and it red-ringed again.

I put tons of crap in my room at Tenpenny Tower and then I go to get some of it to sell and the game freezes everytime! I guess thats what I get for killing everyone inside.

When I try to save it crashes sometimes. Other than that just minor glitches.

i got stuck in vats with the mysterious stranger who kept on shooting a dead carcass. never stopped.

walkin around next to spingdale elementary and jumped on a car... well tried to jump on a car and just flew up like  a hundred feet... soooooo kwl tried to move around.. fell and died from impact...

Out of nowhere one of those cars that's laying around jumps out. Have no idea what made it do that. Made me laugh though.

Aside from occasionally crashing during loads, the only glitch I encountered is that going into Operation: Anchorage automatically kills Dogmeat, which upset me to no end.  Good thing it forces you to make a hard save before starting the simulation, or I would've thrown a fit.

I paid for a room and company, with this Broad called Nova, all she did was lie down on the bed.  No action. 

I went to Agathas house with 2 folowers, Charon and Clover, went iside and started talikng to agatha.

Being stuck in VATS. I had to restart the console and i didn't save so i had to go back 2 hours. Oh, and a AI character got stuck in a wall so i couldn't talk to him, actually that was pretty funny, his arm was just hanging out of the wall.

1. Screen was blurry for an hour. (fixed itself randomly)

the first time i played through it, i had no glitches what so ever, but after i beat it and started a new game, all hell broke loose, the screen started to screw up, it got covered with lines, people started to fall all over the place, robots to...insane

Crashes to my desktop and weird VATS lag that made me have to turn off Antialiasing. That and when I was playing Fallout 3 religiously I would always find that I'd have to restart Fallout 3 every five or six hours or so because the performance would degrade from silky smooth to nearly unplayable slowly. This was a couple months ago, though, and I bet it's fixed by now.

A helicopter randomly fell and killed me when I stepped out of the vault.

It was a nice greeting!

Sea would disappear sometimes. Haven't really had any bad glitches in Fallout.

There was this one time when I was Level 3 at the Super Duper Mart and this fucking Deathclaw and Talon Company Mercs come from out of nowhere when I leave. The Deathclaw took so many fucking bullets.

I went inside a house in Little Lamplight and I heard a noise. I walked into one of the rooms and a guy was lying on the floor with an arm and a leg cut off. No one was in the house... so he must've combusted on his own somehow.

My game has zero sound during dialogue, and i havent been able to fix it yet. Sucks. I have it for PC

Not sure if this is a glitch, but I know it wasn't supposed to happen.  It was during the Little Lamplight quest.  I asked Joseph to bring me to the door that they couldn't get open.  He got stuck on the steps on the way there for about two minutes.  He kept walking but for some reason he just wouldn't go any further.  When he finally started moving again, I found out that my hacking skill wasn't high enough to open the door anyway.

i just got done playing some of fallout 3, and i shot a enclave soldier, and after i did, he fell to the ground, then shot up in the air and his corpse started flying around the room....it was odd, yet amazing

I'm now prevented from entering Tenpenny Towers, the game freezes when I try. I have tried going back to an earlier save and its now  doing it on the earlier save.
I'm thinking that my disc may be faulty, its really annoying because I had a ton of gear at that location.

I used a console command to make 1000 Super Mutant Behemoths and my  game froze! Plus I hadn't saved!

LAAAAAAAAAAAME!

I was finishing the quest Head of State, and a few hours into it, I was about to finish it, when all the people that were supposed to follow me disappeared

The worst would be freezing (happened after downloading all DLCs). Though I think it stopped after I switched from 1080p to 720p. Also dead bodies and some pieces of geometry spz out once in a while (clipping and just flying around like crazy).

Was about to complete the GNR radio quest but when i tried to go back and talk to Three Dog I was locked out of GNR radio forever....the main quest can now not go any further.

its always the ghouls. They stretch limbs through walls and the super mutants like to not die after i decapitate them. Im playing on the PC but i also own it on the PS3 and i have yet to experiance a glitch on the console. 

The worst one I ever encountered was while I was in a fire fight and one of my bullets hit a chainlink fence and then for some reason weird radiating polygons extended from that point on the fence all the way out to the edge of the screen completly blinding my view.  Every time I would look in that direction, even if I was far away from the fence, the polygons would cover the entire screen.  
I saved in a new slot looking away from the fence and then reloaded it, fixed the problem and never saw it happen again.

Mysteriously disappearing followers happened once or twice throughout my three play throughs, but I always save often, so it wasn't a big deal.

I shot the experiment MIRV at my feet and was catapulted into the air. I landed in a barren wasteland with floating plants and wondered if it was heaven?

I shot a Deathclaw with the Tesla Cannon & it shot straight into the air & glitched off the map (only to respawn & attack me).
My most frequent glitch is getting stuck in places I can't fucking jump out of.... or being stuck in VATS.

Most annoying glitch was when i activated my pipboy and the whole screen became super white, and would not go back to normal, so i had to quit to the main menu and load again. Another one would be a robot shooting straight down from the skies, i have had that one happen two times.

Stuff dissappearing from lockers in my megaton house was pretty bad. I had all my pre-war books and clothing those damn lockers.

My first save got corrupted. I had 50 + hours easy in it.

I actually have never experienced a real glitch.

I gues I'm lucky.

I was in the Capitol Building shooting the Behemoth with a missile launcher, and all of a sudden, it launches hundreds of feet into the air, falls back to the floor, and dies. It was amazing, and somewhat relieving, since I didn't have to waste any more ammo LOL

About halfway through the game, I exited Rivet City and went into third person mode.  To my horror, all the textures on every single object in the game was stretched every which way across the screen.  At first I figured I just would never go back into third person mode and I'd be find, but then I realized whenever VATS was used, it would show the hideous graphical glitches in all their glory.
The problem wouldn't correct itself, and so I was forced to reload a save file from about a fucking hour earlier in order to fix it.

i was in a gun battle with some raiders wehn i fell through the road(for somereason there was ground underneath it) and they couldent shoot through the road but i could so i wiped them out.

I got stuck with no followers and a nearby enemy that didn't know I was there, thus no fast-travel. I saved and reloaded and I was still stuck! Had to re-do a whole quest
