Changing Password Not Working

When I try to change my password, the same window is asking for my username and email. I enter these details along with my new password, etc. When I hit make changes, it comes back and says only premium members can change password. So if I leave that section blank and only enter my email and password, it says username not specified.

How can I change my password? Thanks. I'm using the latest safari on the latest OS X.
