Problems when posting reviews/creating lists

This is an odd issue I'm having. Last week I created a list:

This week I wrote a review:

In both cases, after I finished the content and went to post it. I was greeted with a 404 Not Found page. When I back out and go back to find the content, I find that it shows I've recommended it. You can see in both cases for the review and the list. I am most definitely not doing it on purpose. Actually the same thing happened to my DmC review:

I posted that when the new site launched and figured it was just a weird bug...

So I dunno. Not sure why it's happening but I figure I would report it.

So it 404s, then shows up on your profile, with a recommendation from yourself? Does it say "you already voted on this user review" at the bottom of those reviews, without you having clicked on any kind of recommendation button?

Correct. When I manually go back and find that the content has been posted, it shows that I have already voted for it and I never actually clicked on the recommend button.
