In my opinion, I have never heard anything better than Super Street Fighter II Turbo HD Remixs soundtrack, although Shinobi III was way advaced for its time and its tunes kick ass, and Naruto UNS has pretty kick ass tunes as well. What is it for you?
Best Music in a game?
The usuals: Final Fantasy VI, Chrono Trigger, Final Fantasy IX, Dragon Quest IV, Super Smash Bros. Brawl.
The unusuals: Fire Emblem 4, Mystic Ark, Cocoron, and a bunch of other games you've never heard of. I specialize in that.
Chrono Trigger, most of the FF games, Chrono Cross, Grandia II, Legend of Dragoon, and the DQ games.
The best original soundtrack out of any game is Silent Hill 2's, by Akira Yamaoka. All Silent Hill music is good, in fact, and Metal Gear Solid 2: Sons of Liberty's soundtrack (Substance included in that) is a close second to Silent Hill 2. In my opinion, all other Metal Gear soundtracks, while better than average, do not come close to that of Sons of Liberty's. Final Fantasy X was also a very strong outing, due to the fact that Uematsu was almost out, the soundtrack being a collaboration of him and a couple other in house Squeenix composers. Not to hate on Uematsu, but he has the tendency to create a pile of average songs with only a few gems: FFVII OST, case in point. That being said, I have to agree with Shane Bettenhausen that J-E-N-O-V-A is his best song, and one of the best in all of videogaming. Along with Sonic the Hedgehog 3's Ice Cap Zone.

It's melodic and has that awesome MSX synth. I mean, your username even references it!
Plus Uematsu produced more than just a few good tracks per OST. All of both FFIV and FFVI are classics, and if you haven't heard Lord of Vermilion, you're really missing out.
Amon Tobin's amazing work on Splinter Cell: Chaos Theory. After hearing his album Supermodified, I used to wish he'd do the music to a spy movie (no kidding, ask my girl how much I'd go on about it).
Never could I have dreamed that one of my favourite artists would contribute such a deep, gritty and urgent soundtrack to a video game. So good I bought it on CD and vinyl. Amon Tobin is just plain brilliant.
Final Fantasy VIII had me, but very honerable mentions go to; Metal Gear Solid Series and Half LIfe 2.

Also, Rez. Once again, I'm not a big fan of trance music, but I couldn't think of a more fitting soundtrack.
However, if the trailer music is anything to go by, PixelJunk Shooter is shaping up to have the best soundtrack.
As someone who listens to a podcasts that is "solely dedicated to the academic study of video game music" I'm pretty well versed in this subject. For a while I thought that FF7 was "the best" VG soundtrack, but it's really FF6. The opening theme sends chills down my spine every time I hear it. Golden Sun's soundtrack also has a place in my heart, along with FE7's.
If I had to say the BEST 1 soundtrack, I would say Alone in the Dark had the most rocking soundtrack. Whether that game deserved such an amazing orchestral soundtrack is an entirely different argument.
I would also say that the Katamari Series has one of the best soundtracks. Each sequel in the series actually features sequels to songs from previous games. Not just remixes, but actual sequels. That's crazy. Not to mention it's all just really good and artsy.
In addition to the iconic Halo themes, I really loved the OST to Kingdom Hearts, and the last two SMT: Persona games.
When I played WOW, I wasn't overly impressed by the music to the original game or Burning Crusade, however Wrath of the Lich King had some really cool sounds in it.
As far as real life music goes...I would have to say Grand Theft Auto: Vice City. That game has an excellent soundtrack and gets extra props from me for including "Pale Shelter" by Tears for Fears (my favorite song of theirs) as opposed to the more common and obvious choices (ie. "Everybody Wants to Rule the World", "Shout", "Head Over Heals", etc).
As far as original game music goes...I am going to pick Mike Tyson's Punch-Out!! only because I absolutely love the jogging music. That shit pumps me up and is a great motivator while exercising. I'm also a huge fan of the Goonies II theme and think it's a bit underrated for its awesomeness!
