Thats what happened to me, only bought the shield upgrade for the ship and sent the wrong people into leading squads, I think I had like 3 or 4 survivors.
I'm so glad I didn't have that ending, poor Joker :( , although ME3 would be really interesting if you have to start from scratch with just Joker. "
I'm pretty sure that if Shepard dies, you simply can't import your save file. Would be kind of cool if they did it like Dragon Age where you create an entirely new hero, but I think it's easier for them there because the main character doesn't speak.
