In a game with SP and MP, which one do you play first?
Well, I try to finish the single player campaign first, that's for sure. It sucks when you're playing through a game where you've messed around with the multiplayer (which is repeatedly established as separate from the campaign) and instantly recognize "oh, yeah, this whole area was a multiplayer level. Well, fuck, I already know how to get through here!" (I'm looking at you, Dead Space 2, you're the specific offender that comes to mind, though I have to imagine other games have done this.)
Always single player. The only time I have ever played the MP first is when I got Call of Duty 4 as I knew it was great due to being in the Beta. I think it was about two weeks until I got around to the campaign and then beat it in one or two sittings.
What would "other" in the poll constitute?
I play the single-player and usually ignore the multiplayer. Sometimes I'll play local MP, but that's about it.
I use the singleplayer as "training" for the multiplayer. Typically singleplayer tutorials are much better than the multiplayer "tips" you get during loading screens. Plus, by the end of a 10-hour game, you're usually pretty competent at the core gameplay.
This question immediately brought to mind Black Ops or Battlefield: Bad Company 2, both of which I played through almost the entirety of the singleplayer before moving into multiplayer (although it was extremely hard to do for BC2 since I had already played in the beta and knew how much I loved that game).
While it can be hard to not play with friends if you see them offline, I always try to knock the SP out of the way early on, knowing that I might not ever get to it after I get entrenched in the multiplayer for games like those I listed above.
I usually always tried to complete the Single Player campaign first. I did it for MW 1 & 2 however for Black Ops I jumped straight into multiplayer as that was what I bought the game for. I'll probably do the same for MW3 as well.
I always play the singleplayer first before moving into multiplayer. The only exception is when the singleplayer is actually fairly long, in which case I'll probably get in a handful of multiplayer matches before I've finished it, at least if I know people who also own the game. I love online shooters, for example, but they're always more fun if you're playing with some people you know. I'm not really in a hurry to jump into multiplayer if I know it's just going to be me and a bunch of strangers.
I think the only time I've ever jumped straight into multiplayer without making any significant progress in the singleplayer was with Dawn of War: Soulstorm, because... well, let's face it: Soulstorm's campaign is exactly the same as Dark Crusade's, only it's... not good.
Always the single player first. More often then not I'll usually ignore the multiplayer altogether. Even in a game like Call of Duty I'll venture into the campaign before bothering with multi.

Does co op story count as mp or sp? Or both?
With games like Halo/Gears I often dive straight into story on the hardest setting with my cousin, before we even try the MP/horde modes/etc.
I "almost" completely skip multiplayer. If there's local multiplayer, then I may play it. Online though, not really. I'm a free/silver member on Xbox Live and I don't have a working headset for the PS3.
Always single player. I enjoy playing games for the story, and the single player acts as an extended tutorial to get you familiar with how the game controls and functions so when you are online you are not handicapped.

I generally finish the single player before jumping into the multiplayer. I mostly play games for the campaign and if a game has much of a multiplayer component that campaign is probably only 10 hours or less anyway. It's also usually more fun to use weapons and abilities for the first time as they're integrated into the story. Being given some crazy weapon to get through a segment of the game would lose some magic if you'd already played around with it in multiplayer. I also have a constant fear that some idiot's going to shout spoilers into his mic because he thinks its funny.

Edit: The reason being that I will probably not get through the single-player in a timely fashion at all if I play the multiplayer before I have finished the campaign. To me, FPS campaigns are usually the most fun when completed in a few sittings.
Boring ass predictable deathmatch multiplayer that can be found in pretty much every shooter? I skip right over that crap. Fun ass co-op, beat the shit out of crazy ass mobs and score loot multiplayer? Sign me up!
