for me its castlevania lords of shadow, that game has an amazingly beautiful soundtrack that rivals even the lord of the rings soundtrack. lords of shadow has the most epic soundtrack of any video game in my opinion.
What game do you think has the best soundtrack?
Been playing through Hot Line Miami again before 2 comes out. That sound track is something else and really brings that game to a whole other level

Some games not mentioned yet deserving consideration
Xenoblade Chronicles. I dropped $40 to get the CD set of this soundtrack (4 discs) and it was TOTALLY WORTH IT.
Killer Instinct on XBone. Mick Gordon is a Wizard and should probably be added to the game as a proper character, slinging magical notes to destroy the opposition

It's a toss up between Bastion and Hotline Miami for me. Both of those games soundtracks hit me just right and made the games that much more outstanding.

I don't know if I can say they are "best", but both Jet Grind games waged a full frontal assault on my subconscience.
Also, people love to hate on the game so this gets overlooked, but God damn the Final Fantasy XI soundtrack (and all of the expansion OST's) are great.
Hotline Miami, Doom, Super Mario World, Final Fantasy 8 specifically this song (The Man with the Machinegun), Super Mario Galaxy 1 and 2, Final Fantasy 6, Metal Gear Solid 2 and 3, and all the classic Megaman games.
I've probably listened to the Far Cry 3: Blood Dragon soundtrack over 100 times by now. It's part of my music rotation when I write.

I'd probably have a different answer depending on what mood I was in.
I do know that these are among my favorite though:
- Jet Grind Radio
- Hotline Miami
- Persona 3 Portable, female MC path
- Persona 4 Golden
Any of those games almost wouldn't work without the soundtracks they have. They are completely integral to the experience.

In terms of semi-recent games of the past few years, I'd say Kid Icarus: Uprising's soundtrack (apart from being my own personal best game of 2012) was all-around pretty top-notch stuff and very eclectic; spanning from retro-style chiptunes to hard/electric rock to symphonic, big-band, jazz, and a whole lot of other genres/fusions of genres in between.
Just a random track that I particularly enjoy out of many excellent tracks:

Though my vote goes to Chrono Cross or Lunar SSSC, I'm a sucker for that boat scene with Wind Nocturne so that might get the edge but Cross is a close second.
Jeremy Soule's music, Skyrim is fantastic.
I dont know about game overall, but I heard Or symphony preform FFXIIIs liberi fatali during their rePLAY: symphony of heroes concert, and it was jaw dropping. I couldn't believe my ears were hearing it live.
Also, their performance of dragon age's "i am the one" was astounding. The singer had the most beautiful voice i've heard.
Listening to these songs on youtube is just okay, but hearing them live was mind blowing
If I had to choose I single game it'd be Final Fantasy 8's, Nobuo Uematsu's greatest work in my opinion, from the opening FMV onward ever track is just so good.
There's been a crap load of great soundtracks over the recent years, stuff I listen to regularly include Metal Gear Rising's and Hotline Miami, older stuff like Persona 3 and 4, and Nier.
The one I've gone back to recently because it had been forever is The World Ends With You, it's such a strange blend of music that I often don't listen to but for that game was absolutely perfect and super catchy:
This thread pops up at least twice a year. The answer is Mega Man 2 and has been since it was released. Nothing will ever top Mega Man 2. You can stop asking this question now.
Other games may have one or two stand out tracks that are better than some of Mega Man 2's, but nothing has ever come close to being as consistently great as Mega Man 2. Every single track is awesome.
It's hard to call a favorite because every time I listen to them, I have a new one, but I do keep coming back to Wood Man's stage:

I'm just going to place a couple honourable mentions here as I tend to favour soundtracks that not only complement their respective games really well by properly fitting with the game's vision, but stand on their own bringing nothing but imagination. Music is more than catchy tunes or glorified mixtapes (not saying I don't like them as I love most soundtracks people mention in these threads, but I tend to favour more complicated and/or original works in higher regard personally).

If we're talking classically orchestral scores, I really REALLY love Alan Wake. Petri Alanko's music is gorgeous. This is probably my favourite piece:
The Halo games got me into video game music in a big way. Halo 3 is awesome for the big thematic stuff, and ODST is phenomenal at setting the mood.
Darren Korb's stuff on Transistor and Bastion is incredible, as is Austin Wintory's score for Journey.
Anything Koji Kondo does for the Zelda games is on another level.

I'm also sad that no one has mentioned Mass Effect, especially that first game, but the entire series has great music. Suicide Mission, Vigil, even the map screen music is awesome and totally sets the tone for the game.
Shenmue and Shadow of the Colossus are what stick out to me the most, though I certainly have my favorites from plenty of other games, too.
@fram: The Poets of the Fall songs in Alan Wake are also pretty great. Big fan of The Poet and the Muse.
Persona 3's soundtrack is amazing. So many emotional tracks. Dang it, now you've made me want to brush the dust off my PSP and play some Persona 3 Portable.
It's super cliche, but whatever, Final Fantasy VII's score is still far and away my favorite. I'm also hugely fond of Deus Ex: Human Revolution, Mass Effect 3, Halo 3 (and Reach), Journey, bits and bobs from the Assassin's Creed scores (I've gotta remember to look up one guitar centric song from Black Flag, the one that plays while exploring a bog near the main town), and the Monkey Island games.
