I think that you could get a permanent ban right off the bat, but you would probably have majorly broken the rules. (Example: posting porn everywhere)
A person can appeal a ban by getting in contact with a staff member, although that is a little difficult to do.
For your last question, I have no idea how the mods handle that.
Be a jerk and you'll be well on the way. There are some very notable examples, though.
- Posing dubious proposals and questions, such as whether a sister's dress is too sexy and would you like to buy it.
- Spamming pointless threads, particularly with material that infringes the site's rules.
- Spamming derogatory threads with multiple accounts. That one might even get you an IP ban.
I'm sure there are plenty of examples to find if you search the old threads enough.

Also, @deathswind:you shouldn't have anything to worry about unless you know you're going to do something against the rules. I've never worried about being banned because I know I'll never walk that line.
A lot of misconceptions and misinformation being thrown around in this thread, so I'm going to nip it in the bud and lock it.
To address the basic questions of the OP, an immediate and outright ban on a first offense is something that very rarely occurs in only the most extreme cases. In general, users get several opportunities to correct their behavior prior to a ban by way of multiple warnings and suspensions. In cases where a person feels they have been wronged by the moderation process, they can appeal to the staff.
