You never dive, that's what I learned. Diving leads you into a possible losable situation in which you are much likelier to get shot at. I generally stick behind cover in the left side of the room (Same way I enter rooms) and inch out shooting everyone (With a single pistol or ingram) that comes into the field of vision. Slowing down time if overwhelmed. Works every time I do it.
Although it always helps when you know where everyone is before you get into the situation.

Call of Duty 4: Modern Warfare turned on Veteran mode. I can complete Mile High Club on Veteran within half an hour of trying but the rest are just ridiculous.
Ninja Gaiden on xBox, not a doubt in my mind. Getting to Alma was a chore in and of itself, having to beat her is still a tremendous task. The levels themselves get harder (I HATE GHOST FISH!!!) but thankfully the bosses get easier. I hate respect you Ninja Gaiden.
skate and skate 2 can also be really frustrating, some tricks are so fucking similar it makes it half impossible to complete some of the challenges. and the film challenges in the original were the devil spawn.
Any game where snipers can just dominate. It really pisses me off. Also, Mirrors Edge really frustrated me with how the combat was really horrible, yet i was still forced to do it.
Off the wall, but DBZ: Burst Limit is a frustrating fighting game for me. I'm 100% fine with losing legitimately, but Burst Limit has too many cutscenes in the middle of fighting, and when you come back the camera is rotated in all sorts of directions -- it really sucks sometimes. Shame, because it's an exciting fighting game to play otherwise.
Also, the comment about Braid up there somewhere is pretty laughable. XD
Edit: Oh, honorable mention to Street Fighter IV's training challenges. Oh. My. Science.
Hmm, I guess Mirror's Edge pissed me off in some levels, LittleBigPlanet in some of the later levels was pretty annoying when I died a lot, and Burnout Paradise got frustrating when I'm driving waaay too fast and I crash into a wall that I didn't see.
See, in most JRPGs, you usually have to fight a bunch of weaker enemies; it's tedious, but it's easy. In Persona, you have to fight a few stronger enemies. It's usually a lot more fun, but when you die after being in a dungeon for 45 minutes, leveling up and getting rare items, you just feel like strategically dismembering your controller.

olol play lone wolves at level 45... It will give you stress related mental illnesses. at first it seems like your doing great then suddenly your playing goes right down the crapper and you spend 5 whole minutes without a single kill and dying constantly and all you can think is FUUUUUUUUUUUUUUUUUUUUUUU- and since you're angry you're losing concentration and as a result you play worse and worse getting madder and madder until you just think oh fuck this and get a sandwich
