Probably the plane crashing in Left 4 Dead on Dead Air. Despite it being a ruined airport, I didn't expect a plane to crash.
I assumed this thread was, "Holy shit that actually happened." moments, not.. holy shit graphics!

EDIT: Also, Tai in Gears of War 2.  People who have gotten that far in the campaign will know what I'm talking about.  Holy fucking shit.
