A republic isn't simply a form of democracy, and we do get to vote on specific issues, from time to time.
There isn't enough power given to the people in most so called western democracies to call themselves "true democracies". At best you get a choice between this or slightly this. Legislation and true power is in the hands of the executive and judges as well as those that can gain access to them, i.e. corporations, lobby groups and people with money and vested interests. At times it feels like a sham but no one wants to admit it.

Certain states in the US could be construed to be democracies (the ones that allow public referendums to decide amendments to the state constitutions), but the US itself is not. In fact, the US Constitution specifies exactly the words: "Republican Form of Government." And the country as a whole never gets to vote on specific issues.
As for why people use the word "democracy"...well one is because many people are pretty stupid about matters governmental, but mostly because "democracy" as an ideal is a lot easier to sell to countries than "republic," and we've been doing a lot of such selling since European feuding almost blew the world up a couple times a while back.
Really, as long as you don't use it ignorantly, (like claiming you have some mythical "rights" because "this is a democracy!") I think the word is fine. Close enough.

1. A system of government by which political independence is retained by the people and exercised directly by citizens.
2. A constitutional republic where the people have a voice through their elected representatives.
By the second definition, America is a democracy. It's also a republic.

If you read in Wikipedia, neither is a subset of the other (there's weird exceptions like some countries elect lifetime kings, etc.), though most democracies are a form of a republic.

No, a democracy/republic only states how people "are put into office," not the issues themselves. For example you can have these types of democracies (quoting Wikipedia):
"Representative Democracy involves the selection of government officials by the people being represented"
"Direct Democracy is a political system where the citizens participate in the decision-making personally, contrary to relying on intermediaries or representatives"
