When I was a really young lad, I played a game based on one of the Rugrats movies. Now that was a terrible game. I forget the name, but it was on N64. 
 
But then again, I also played Superman 64, and that was pretty bad... 
 
Edit: I also got PacMan Fever for Christmas once. That was pretty bad.

Oh wait, that I've played?  DefinitelyDead or Alive: Xtreme 2.  I've also played Superman 64 (I was a kid, so that doesn't really count) but man the DoA Xtreme games are some shit.

Armorines for N64.

Superman 64. Original, I know.

From now on Big Rigs doesn't count as being the worst game you've ever played, that just makes all of these threads boring as can possibly be.
 
Dynasty Warriors for PSP, or Sonic Adventure.

I've beaten E.T. for the Atari 2600.

The original Vigilante for the TG-16.  I downloaded it from the VC on a whim, and damn it sucks.

Herzog Zwei.  (throws up in mouth)

Most recently i went back through my playstation catalog and played Call of Duty 2: Big Red One. The opening level bugged out on the bridge scene and the entire bridge fell through the level making it unable to pass. I shut my playstation off and said "oh my god that was awful"

Soulcalibur Legends. God that game was bad. just one of those fairly early shitty wii games that had nothing to do with fighting. I even knew that and bought it... huge mistake.

Custer's Revenge. Probably the lamest game ever made. Those programmers at Mystique sure knew how to get the most out of the Atari VCS.

Off the top of my head...Superman 64 and Tamagotchi Party On.

Back to the Future for the NES. It's so bad and I used to play it all the time when I was a kid.

E.T. I played it just for it's badness, and well it's bad.

I don't have a lot of games I hate, I always choose wisely when I make my purchases, other than the days where I'd rent games, I'd rent anything, and since they're like 2 dollars, I thought they were worth it, other than this game:

In B4 Halo

Army of Two - it was just dire. So many things wrong with the gameplay.

Definitely Dai-Katana. I remember playing it as a kid and just thinking that it was way too hard, and that nothing made sense. Eventually I came back to it thinking that it had to have had some merit, and yet I was wrong, so wrong... you're lucky if you don't get killed by mosquitoes at the beginning of that thing!

Paperboy for the Nintendo 64.

Battlefield Bad Company 1. I have nothing good to say about that game. Stupid plot, annoying characters, shitty game play, some of the worst AI I've ever seen.
