I'm guessing that thematically this is Kojima's white whale. The game he's been chasing his entire career, the toil of which has left him scarred and disfigured. Bonus points if the game contains extensive cutscenes detailing the physiology of various species of whales
Yo... Um. Dat doctor in the beginning was Madnar. It was certainly MG related. If you look at the crawl animation where he's flopping around, it's way too similar to the opening demo scene in Ground Zeroes of Big Boss crawling.
And, Liquid Snake lost his arm. They used his body as a substitute for Solid Snake after the tanker went down, but, as quoted by the doctor in the beginning, "You've been in a coma for... quite some time." HOW'S 10 YEARS SOUND? Also his hair is blonde that is starting to grey. It's the same mullet as Liquid Snake. British flag in the background? His arm is gone so there's no tattoo, seeing as how Ocelot was the temp owner of it.
@sodacat: There's also the fact that Kojima was working on getting Liquid Snake's side of the story into Peace Walker too. They originally planned that game to span from the 70's to the 90's, and hey, who's to say not even further?
Could it be that this is that side of the story? Liquid Snake was with the British SAS when he was captured in Afghanistan during the Gulf War, and went AWOL after that, turning up later to take hold of Shadow Moses Island. He was a good guy before MGS1, and Kojima wanted to include what made him go bad.

Big Boss was also forced into a perpetual coma until Solidus was "killed" in MGS2 which takes place in 2009. Then they used the body parts of Liquid and Solidus to reconstruct his body. So it would be possible for this to take place between 2010 and 2014 except for the fact that Big Boss looked really different at the end of MGS4 and also had both arms. Which to be fair he could have gotten another one after the ordeal finished. This could be when Big Boss is put back together and he was transported out for safe keeping from the Patriots but obviously not too safe.

Your forgetting that Liquid had both limbs in MGS1, and even if it takes place after it would have been the wrong arm if I'm not mistaken.

It's the wrong arm though. Ocelot used Liquid's right arm.
I really hope they spin this out into a game instead of some viral marketing demo for another MGS game. I mean, I'm excited for the new Metal Gear and I think this is a cool way to tease it with all the clues, but at the same time, the trailer sold me on a game where the protagonists are a one armed man and a man whose head is veiled under bandages in a F.E.A.R. like environment. I mean, a troop of armed, voiceless soldiers acting with the single determination of eliminating helpless rehabilitating soldiers in a hospital was genuinely frightening.
