First time through I only lost part of the crew. The squad survived. In fact, in all three play-throughs, everyone in my squad survived and in the last two, the crew survived. On the character I've made for my 4th playthrough, I'm going to intentionally make some poor squadmate choices

I lost Jacob, I was gonna send Tali down the tubes but there was no way I was gonna risk her getting killed...so seeing as Jacob wanted to do it, I let him do it! I would have cried if Tali died...

I lost Zaeed in the second fire team decision. He comes through the door injured and dies. Im like "meh". He was funny and an interesting character, but I didnt use him all that much. Canon wise, I rationalise it as he was an old war veteran with alot of demons, he hints at "Ive done some bad shit" , he wouldnt have minded dying on the battlefield for all of humanity.
And Jack? I didnt have her loyalty. I originally lost Mirandas, then when I realised what Bioware were doing, I instantly reloaded and lost Jacks. Because it annoyed me that you cant pacify them both because of a stupid meter, so if I had to lose any1, it would be that eyesore, earache of a character, Jack. Didnt mind the character was in there, but I would prefer sweet tits in Mass Effect 3. She was really haunted by her childhood and seemed to be searching for some peace, so I rationalise, that maybe this is it.
Saved the entire crew otherwise.
Decisions I made were:
Upgraded ship fully
Tali as tech - Best tech
Garrus as Leader - He lead that elite squad
Thane as escort - "Can adapt to almost any situation"
Zaeed as second leader - War Vet
Samara as biotic - "Incredible" Biotics
Then I took Samara and Garrus as my final 2 into the final battle. I wanted to make sure that if either of my fav characters lives were in the balance, I had direct control of them.
I also like Legion. Hes a really interesting character. It was a toss up between the 3 for the final battle.
My entire squad survived, but I lost everyone on the crew except Dr. Chakwas.
Next time, I'll make sure I'm ready to go through the relay immediately after they get abducted.

Nobody, not even the crew so far as I know (more than just Chakwas was there), I went through the relay like 45 minutes after having the ability.
I chose legion to go through the ducts, garrus to lead the attack squads both times, Jacob took the survivors back, and samara is really the only choice for biotics.
I took my usual pair of Tali and Miranda with me through everything.
Oh, and I blew that mother fucker up. TAKE THAT, MARTIN SHEEN!
I don't kncan't quite finher how anyone would die during my ending, I had all the loyalty and upgrades. But for some reason during the shielding part, Samara died. I was confused. But then I got word that the entire Normandy crew died on their way back to the Normandy, because I neglected to give them an escourt out. lol. I think the two occurrences might be linked. Other than that, I have no other explanation. I'm interested to see how ME3 will start off with a whole new crew and Samara missing from the universe.
Going to finish my other ME1 save from last year and import it and start a new ME2 playthrough, if for anything, the ending alone..
When there's spoilers involved, most gamers would consider writing (SPOILER) next to the title of the topic or use the spoiler icon if it's for posts. It doesn't mater anyways, Giantbomb's going to be flooded with pre-adolescent immature gamers anyways, just like Gamespot." @Alphiehyr: In almost all the pre-release hype they established that this was a suicide mission, and not all your crew will becoming back alive. The doctors said that durring E3. And even in the pre-release Bioware Mass Effect interview. I also waited 2 weeks after release to launch this poll. And intentionally didn't leave "Everyone died including shepard" as an option. I really am sorry that I ruined it for you, but maybe you shouldn't be cruising around the mass effect forums if you haven't beat the game. Other Forum Posted Titles. It was up to you to stay off of gaming message boards if you wanted to preserve the experience. Hilariously, this is exactly what I did, and yet I still had the ending ruined, because I was stupid enough to try to figure out which achievements I was missing prior to beating the game. The developers are at fault here, not gamers at large. "A game has been out for two weeks doesn't mean everyone will have the copy. The board isn't built purely for spoilers and if it was, they must be stupid putting the little spoiler feature. I browsed around Modern Warfare 2's board for a week after the game's release without being exposing to any spoilers because people that posted there were kind enough to write (SPOILERS) next to their topic.

I feel that the developer said durring hype of the game that depending what you do your team may die. That's why it was even written in the achievements. And I really didn't spoil any thing, because maybe none, or all of Alphiehyr's characters would die. His game is still to be determined. I never told him how his game will end.
Just Tali...I was genuinely distraught...for a full 5 minutes. I have subsequently gone through again and everyone survived. I'm tempted, on my next playthrough, to intentionally botch the suicide mission completely. No ship upgrades, no loyalty etc. Just to see how it plays out.
Noone died and I blew up the Reaper in my first playthrough. My Insanity playthrough Legion died, but he wasn't loyal. On that one I didn't blow it up.
Only Kelly and Miranda died. I never got enough Paragon to make nice with Miranda after she fought with Jack.
one died on my first time through. it was tali who died after having a large pillar fall on top of her after defeating the reaper larvae. i kept the base.
