Ghouls, they are Ghouls not Zombies. Chances are the mutation will be like in the previous Fallouts where you grow an extra toe preventing you to gain access to a certain town or pass a physical. But then again Bethsoft has dumbed the series down to meet the below average IQ of console gamers.
