Luci Fulci, Dario Argentento and other italian, exploitation directors of the 70's and 80's certainly had a way with gore that's hard to find anywhere else. The murderscenes in Susperia and Inferno and the fucking insane vomit-scene from The City of the Living Dead in particular comes to minds.
The rape scene in A Clockwork Orange both scared me and was what sparked my interest in movies... I just couldn't believe you could show that shit on screen. Speaking of Kubrick, the typewriter scene from The Shining always freaks me the fuck out for some reason. That one's hard to explain though.
I've seen Salo and found that to be more upsetting than disturbing in a good way. The intentions behind Salo are fine and you really feel the directors hate towards the fascists, but the violence ends up feeling super cheap, exploitative and unearned. The only reason so called movie-buffs seem to like it is the pretty scenaries and the fact that it's about "hey-you-guys-fascism-is-like-super-bad".
Other movies worth mentioning are Evil Dead (2013 or the original, take your pick), Caligula (if you havn't watched Caligula, oh god, WATCH CALIGULA!), Wolf Creek, Mulholland Dr., Pontypool and Straw Dogs. Straw Dogs may be the most uncomfortable I've ever been watching a movie. Black Christmas really fucked me up to (the original, of course, not the awfull fucking remake).
