Not sure how rare this is, but I have an original Ultima IV box (for Apple 2E!) with cloth map, faux-leather guidebooks and all that stuff.  There was an ankh in there at some point but I'm pretty sure I lost it years ago.  My dad bought this when I was 4 or so.  His hand written notes are in there too :P
