That screencap is fake. If you watch Miley's performance, you can see Will Smith and his family enjoying it. The screencap was taken during Lady Gaga's performance...I think... (that's her in the bottom right). If you went to MTV.com during the show, you could choose an audience cam, and the main show would play in a window at the bottom right of your screen.
