Keeping in mind that this is mostly pretty subjective, I think that, when I consider both technical and artistic prowess, The Witcher 2 is the best looking game ever made. It is truly a PC game, although they did hold back somewhat because of the consoles. That being said, there are settings in that game, specifically ubersampling, that do ungodly things to your PC. The lighting model, the polygon count, the textures, the particle effects, it's all extremely well done in comparison to the competition. CD Projekt have always referred to the quality of the graphics in the context of RPG games. For an RPG, they think it is the best. I would agree with them. That shouldn't be controversial, though I am sure there are some out there that prefer some other modern RPG's graphics.
I am currently playing Skyrim, and, although it is a very nice looking game with many downright gorgeous moments, it is very much a console game built using technology from 2005. The Creation Engine, as noted on the Bombcast, is fundamentally Gamebryo. Bethesda has iterated from Oblivion to Fallout 3 to Skyrim, each successively besting that which came before it, but not by orders of magnitude. The Witcher 2 compared to The Witcher, is an entire generation of difference.
I think it is probably the best looking game ever made, but that judgement is based on more than the technical. It is the fusion of great tech and beautiful art that makes The Witcher 2 the greatest looking game available. Guild Wars 2 will soon usurp it!
Note: On pure technical terms, Battlefield 3 is really amazing.
