Some games have a health bar like normal and is depleted like normal, but when in critical condition your health will regenerate which will then stop at a certain point. Once this has happened you must grab a health pack to continue any further in your healing process.
Some will have a regenerating shield only (Halo 1), the shield acts as a buffer to keep minor damage from adding up, but when depleted from heavy combat you are left with a normal health bar. Again, only restored from health pickups or checkpoints.
The last kind of game will have no segmentation and your entire life supply is regenerating (Call of Duty 4). This means when walking around in non-combat areas you are always at full health. These games have no need of health pickups, and often do not even show a health bar of any kind. In such cases your closeness to death is mostly determined by indications of your character's pain, which often times will make the screen go shades of red, or make it blurry, until a set amount of time has passed without taking any damage.

How Regenerating Health Came to Be
Halo or Halo 2 are often credited for the first appearance of regenerating health, though this is not the case. Though Halo was the game that made it popular amongst developers and gamers, the first appearance of this popular method of health control was actually in the game Wolverine Adamantium Rage.
