Suicide button. There's times where I just wanna watch my meatboy die in comical ways. Would be good when you mess up a speedrun and simply can't wait to off yourself.

In replays, I'd want the meatboy who completed the level to be a different color than the silhouetting dead meatboys.

They should add Viridian from VVVVVV as a playable character, if for no other reason than it would totally break the game/be impossible to complete any levels as him.

It would be awesome if it automatically shared/downloaded the ghosts from your friends. 
 
Exploring that idea more, I think it would be really cool if you could directly jump into your friends "worlds" and see their progress. Nothing would motivate me more then seeing that my mate has gotten this bandage or cleared this dark level.

anything this game has to offer, i will take it, 1200 points for a 7 second map, i would take it lol

Word.  More weird-ass characters from other indie developers is the only answer, I think. And more levels.  EDIT: Play as the developers and kill them. Kill them lots. "

Meatboy should always run, with the trigger to make him walk.

Dr. Fetus as a playable character, or maybe just one chapter where you play as him. Touching anything that's not cold, dead, or made out of metal causes him to die. Just flip the game around to be absolutely crazy.
