Now, just to say this  is not photoshopped i dont even know how this would be done. Ill just quickly explain how this happened, basically i came out of the teleporter but the teleporter exploded as i came out of it.

It's called telefragging. Happens every once in a while when someone else is standing on the TP or something when you are trying to come through the TP. Something along those lines.

Yes, it happens, but only with enemy teleporter exits. It's a simple way to prevent models being stuck togethor. However, it should never happen if you are going through the teleporter.
