The Boomer has been heavily mutated by a strain of infection. It has become extremely bloated to the point that a slight rupture in its flesh will cause it to explode, sending the bile, blood and contents of its body everywhere. It is belived his bloating is caused by the decay of body tissues, creating a build up of gasses within his body (Which explains the small loop of intestine sticking out where it's belly button would be). Massive growths have appeared along its skin, indicating that it may still be expanding. It has developed the ability to project its vomit great distances, and its bile has developed the unique charecteristic of attracting hordes of infected, possibly through smell. Though its mentality is simiral to that of the typical infected, it is not distracted by such things as the flashing lights of pipe bombs nor is it apparently attracted by things such as car alarms going off.
A hideously obese Infected covered in large boils, the Boomer has the ability to vomit on players. When this attack is used the vomit may cover more than one survivor. By using the vomit, the Boomer attracts ordinary Infected towards the covered player(s) and makes them more susceptible to attack, while also obscuring their vision. After being killed, the Boomer explodes and covers any nearby survivors with his vomit.
The Boomer is a playable class in Versus mode. His primary attack is his vomit move, which can be used to coat any survivors in bile and attract infected to them, though this ability has an extremely long recharge time, and his secondary attack is a basic swing attack. The Boomer also has the lowest health of any of the playable boss Infected.
The Boomer is highly susceptible to a survivor's melee attacks, when shoved he will lose his balance and stumble back a considerable distance. This is the best way a survivor can avoid being covered in bile when surprised by a Boomer too close to shoot; by pushing the Boomer and then shooting it from a safe distance. Although you can push him around a few times, enough melee hits will pop him.
In Left 4 Dead 2, The growths on The Boomer appear darker, and The Boomer himself appears nearly bald and a slightly larger than in Left 4 Dead. It has also been revealed that there will be a "Female Boomer" due to its clothes, chest part and hair(although the hair might be growths similar to The Boomers). The "Female Boomer" is still the same infected the only difference is the gender, but in the future this might change. In
recent videos the "Female Boomer" appears to be unfinished as it is far less detailed than the over special infected counterparts. Also new when a Boomer dies the bile now spreads a small pole it is still unknown if it attracts a horde.

There are a few achievements associated with the Boomer in Left 4 Dead. These include:
- Blind Luck: You or another Survivor take no damage after being vomited on by a boomer.
- Clean Kill: Shove a Boomer and then kill him without him splashing on any Survivors.
- Stomach Upset: All Survivors complete a campaign without being vomited on by a boomer.
- Outbreak(Only for PC): Catch a rare strain of infection, then pass it to someone else.
- Barf Bagged: Cover all four Survivors with boomer bile at once.
- Slippery Pull: Smoker pull a bile-covered Survivor until you hold him during Crash Course.
