I assume you mean the actual old lady and not the creepy dude who hangs out there and looks like an old lady? You just miss out on dialogue and sedatives. Doesn't matter.

I guess I'm referring to the writhing, hooded thing on the floor asking me to send people to Oeden. There's an old lady survivor? I probably missed that. But I did encounter some skittish guy near a pile of bones on an awning in the Forbidden Woods. It was shortly after he arrived at the Chapel that the hooded thing vanished or died or whatever.

Old Lady doesn't really have anything, Oedon organizer guy has an okay rune but you should feel pretty horrible if you actually kill that thing, that's like the nicest NPC in the game.

I may have killed everyone in that cathedral on the second playthrough. So no fear.

There's no reason to kill her, she's a free source of sedatives.

I killed him. I needed his rune for my Arcane build. I did feel bad about it.

I found her before i'd heard about the Clinic or the Chapel, and then after finding the Clinic I was like "Oh, hey! I solved the puzzle! I found the place for her to go!" Didn't realise it was a multiple choice thing... sorry, lady.

Eventually she starts confusing you with her husband/lover/significant other. She then starts giving you sedatives if you exhausts her dialogue and you can just "milk" her for all the sedatives she has until she has nothing left. You can just keep going back to her every now and again for more sedatives.

Actually at one point she is gone from her usual place and you can find her dead body, right outside the doorway she sits by, with a note left for her loved one. She actually dies going out looking for sedatives for you.
