-Also, Paul Rudd and Seth Rogen were playing Mortal Kombat: Deception in The 40 Year Old Virgin.
-Simon Pegg and Nick Frost played TimeSplitters 2 in Shaun Of The Dead.
-You could see some people playing a Hitman game in an apartment, in the Hitman film.
-In Role Models, Ronnie and some dude play Fifa 09(?) at a party.
-World of Warcraft, GTA Chinatown Wars, CoD: World At War and Guitar Hero have all been in South Park.
-Rock Band is played in Paul Blart Mall Cop.
-Super Mario Bros 3 was in The Wizard (most obvious reference here).

I don't think this really counts but hey ho - I'm an avid watcher of 'The Apprentice', a television show that we have over here in the UK (not sure whether you'll have heard of it). Anyway two episodes in a row they randomly used sections of music from The Sims 3 and Fable ..... very out of place but weirdly it kinda worked!!!
Not sure if this counts but they are a good band, in the new Manchester Orchestra music video for "Simple Math" a OG grey brick Gameboy appears twice in the beginning
Someone may have mentioned this, but Charlie's Angels infamously featured Final Fantasy 8, where two kids were playing it at the same time, hammering on the buttons in a battle sequence of the game.
Sorry if someone already mentioned it (I tried to read all 16 pages... really, I did), but the convenience store in "Grosse Pointe Blank" has a "Doom II" cabinet (and the clerk plays it while listening to "Ace Of Spades" by Motorhead in one scene).
Terrible movies incoming:
Just Go With It: Adam Sandler's fake kids bribe him into buying a PS3 and PS3 controllers are seen in later scenes.
Hop: E.B. the talking easter bunny plays Celebrity Skin on expert drums in RockBand 2.
Couples Retreat: the entire movie more or less centers around Guitar Hero. Vince Vaugn's character is the creator of Guitar Hero and at one point, he duels a hotel staff member at Guitar Hero so he can get to the singles island.
The smurfs: the smurfs are seen playing Guitar Hero.
Bridesmaids: while the girls discuss where the bachelorette party will happen, Bulletstorm can be seen being played in the baclground on 360.
Iron Man: while Tony Stark watches the news about tereorists killing people with his weapons, a wii can be seen under the bottom left corner of the tv.

That was NHL All-Star Hockey for the Saturn. I'm a huge Kevin Smith fan.
As am I, and nice Avatar btw. I just saw a live performance of Jay and Silent Bob get old a few weeks ago, it was great. I've also heard Smith say that he actually wanted to use a 2d version of NHL but had to go with SEGA's because of some contract. I always assumed he meant like NHL 93 or something for Genesis. Those games were fantastic.
Over Thanksgiving weekend, Edvard's butler is cooped up playing the first Halo in their dorm room in The Prince and Me.
Not technically a "video game appearance" but there are posters for the "xbox 720" in "Real Steal" in the final fight stadium scenes.
Half life is played in the TV show Lost.

Missile Command (Arcade) in T2: Judgment Day (1991) The scene in the arcade where John Connor meets the Terminators for the first time. Also visible in this scene, Rampage.
A few minutes later, Connor is seen playing the sit-down motion version of Afterburner.
OutRun (Arcade) in Donnie Darko. (1999) at least I'm pretty sure it's OutRun...from the scene where Donnie and his girlfriend are in the arcade
