Personally I can't really narrow it down to just one thing... but I'm pretty proud of my 29 kill streak in Call of Duty 4.
Getting an Overkill/Extermination in Halo 3 multiplayer was pretty nice considering I'm just average in it.
In World of Warcraft I once picked up our dropped flag, returned the enemy's flag, and captured our flag all in the span of about 2 seconds. That was pretty awesome.
I also quit that game after 2 years, which is also apparently an achievement I guess.
Well, there was this Pizza Hut in town, and ever since I was a kid there was a sit down arcade cabinet dealio with Castlevania in it. Now I had played the NES game but there was something completely different about going to pizza hut, getting some pizza, and asking your parents for quarters to see how far you could get in Castlevania. Anyways, the farthest I ever got on that machine was the 3rd level, just before the boss.
Now fast forward to last summer, me and some friends charged over to Pizza Hut with a bag full of quarters intent on beating it there. It was affectionately dubbed "Castlevania Summer" because we had played a majority of the games to completion that summer.
Now the twist comes in the arcade version of the game that was at this Pizza Hut. We soon realized why we mega sucked at the game when we were kids. It was set so that you could only get hit 3 times, and the time limits for the stages are ridiculous.
But we did it, and ate pizza, and it was awesome. I think on our successful run we made it to Dracula with only one hit left and no holy water forcing us to actually fight fair.
Pokédex of 250 in the original games, went on it a few weeks back and my play time was 286 -.-' so my addiction started age 6, mind you i was in hospital with nothing to do..
Recently though.. nothing difficult, but grinding to get EVERY car in forza 2 (but that just takes a while)

I played as a sniper in Bf2, and the enemy commander had spotted me on his radar. Some combat helicopter started flying towards me in the distance, and the gunner started shooting towards the cliff he knew I was seated on long before he could physically see me. I knew I was going to be owned soon, so I took a quick shoot towards the direction I could see the bullets coming from. The shot ended up hitting the chopper pilot right in the head, causing the helicopter to spiral down and crash right into the ground with the gunner inside. He was furious, at first he thought the pilot was just trying to show off his "close to ground flying skills", but he was actually dead.
It was the most awesome moment I've ever had in any game, and it makes me furious to this day that I didn't record it.
I was pretty pleased with myself when I beat Ninja Gaiden. (the original Xbox version, not Sigma which was a LOT easier).
Collecting all the jiggies in Banjo Kazooie 3 was also a massive undertaking.
I also have one of each type, of all the Pokemon lined up in my boxes on Pokemon Emerald (minus Mew,Celebi and Deoxys).
Well I got 5 stars on all the songs in guitar hero 2 on expert.
But I got Final Fantasy X when I was 11 or so and never played an RPG before so I didn't know how to play. And i kept coming back to it over and over again getting farther but always getting stuck. So I quit it for a few years and came back and finally beat it. It only took 6 years.
100%ing F-Zero GX on the gamecube.
Beating Halo 2 on legendary was my real crowning achievement for the XBOX.
Being the best map maker and combatant in RUNE for the PC.
Getting all the medals in Star Fox 64.
On the PSX I guess beating ROLL AWAY was pretty impressive considering I was like 8 years old.
The first time I whooped Bowsers ass in Yoshi Safari on the SNES.
Completing that godawful Tiny Tunes game for the NES of course.
