I generally do up all my buttons, except on some of my smaller shirts. People occasionally mock me for this practice, but it makes me feel more secure somehow.

Not always, but most of the time I don't. It all depends on the way the shirt falls on your body. Some shirts look great buttoned up, some require less buttoning to look good. I guess it's in the stitching.

I only wear shirts with buttons when at relatively formal things, so I usually do up the top button.

I find doing that hides my bling so I don't, even when I wear a tie, it also helps bodily scents not stay trapped in the shirt, to have to top one open and one of the scents easy to pick up at a busy club is sweat, so f that.

I do tuck it in though.

Shirts with a collar typically have a button on the collar. Do you consider that the top button? I don't. I never button that one up.
 
The button below the collar line it depends. Usually it's undone but some shirts it looks alright to button it.
