Do you do up the top button on a shirt?
I generally do up all my buttons, except on some of my smaller shirts. People occasionally mock me for this practice, but it makes me feel more secure somehow.
Not always, but most of the time I don't. It all depends on the way the shirt falls on your body. Some shirts look great buttoned up, some require less buttoning to look good. I guess it's in the stitching.

I usually wear a t shirt under my button ups, so I end up leaving two or three buttons undone.This too. I never button up the top two or sometimes all three buttons on a polo or button-up shirt since I wear a tank-top under it.

This also, if you are wearing a shirt underneath maybe more than one button can be unfastoned.
Never, unless wearing a tie. Even then, if I'm feeling rebellious, I'll keep it undone with a tie and go for a rakishly dishevelled look.
People who button the top button without a tie look stupid and the practice is often associated with the mentally impaired (or hopelessly nerdy).
