There's an American visiting at our office and every morning I get a -"How do you do?". Now I'm Swedish...and asking how someone is isn't a greeting phrase here. We only ask that if we actually want to know, so our gut reaction is to be honest resulting in C. But I seem to recall Americans (possibly Brits too) have a fairly standard social behavior when it comes to this phrase. Help me out here?

You just ask the question back. It's synonymous with "Hello," for some reason. "Fine, you?" is also good. Actually, all the options are totally legit except C.

The proper response to 'How do you do?' was a reciprocal 'How do you do?', as in this exchange from Oscar Wilde's Lady Windermere's Fan, 1892:

Lady Windermere: How do you do, Lord Darlington?

Lord Darlington: Who do you voodoo, bitch?

Lady Windermere: *gasp* Lord Darlington! You have most pleasantly wibbled my frusset pouch, sir!"

Sometimes its rhetorical (they dont actually expect a response), but if you said "I'm fine how are you?" that would be fine as well.

Usually it is "How are you" in my part of the country, but depending on how i really feel, I will say "Great/fine, how are you?" (which they will respond with a similar "good/fine" usually and that will be the end of that) or if I am feeling particularly shitty that day I will just give a "Hey" back. To Americans it really has nothing to do with how we feel or wanting to know how someone else really feels, it is just a standard greeting.

edit: ironically, he probably thinks that YOU are weird for actually answering the question lol

It also depends on the answer, really. Like a summary, "Good! The kids are doing great." is alright. Anything beyond that and we Americans start wondering if we've fallen into some kind of alternate universe or something.

C is the correct answer. They're either being polite or want to talk. If they're being polite, knowing you'll drive off on some tangent, they'll stop asking. Otherwise, you make a new friend. There you go.

ok or alright, i never say how are you doing back. im probably a bad person.

didn't know this was just the specific "how do you do" i thought it was "how are you doing" "how are you" etc.

barely ever get that, and i'm glad.

It's not exactly verbatim "How do you do", but the gist of it is that he's asking me how I am doing. And like I stated, my gut reaction is to actually tell him how I feel. So far I'm sticking with -"Good, good.".

I just find cultural differences like this fascinating.

Honestly I answered C. If you ask me how I am but only wanted to hear a single sentence of redundant conversation, you are defeating the purpose of asking 'how someone is'. You are also wasting my time when I have better things to do. The word 'fine' is used too excessively that it means nothing to me at this point.

Hey now I'm swedish aswell, and i say "hur är läget?" (wich means is basically the same as "how do you do?") 
Alot of my co-workers (and friends) say this also. 
 
And for me its really just a way to be polite. Then again, I wouldnt mind an honest answer.

Hmm, this is true. It is much less common though, and you often do get an honest response. For example if someone has a cold they'll say so.

It's a good combination of old style question/question and a more modern question/answer.  

Honestly? I probably ask my co-workers every day how they're doing. I don't know if it's a Pittsburgh thing but we tend to ask how the other is doing most of the time whether it's a friend, co-worker or a store clerk. Then again, maybe I'm the only doing it and I'm freaking everyone else out.
