After a violent death by an unknown force and a timely reanimation by the human supremacist organization Cerberus, Commander Shepard must assemble a new squad in the seedier side of the galaxy for a suicide mission in the second installment of the "Mass Effect" trilogy.

For those that had everyone survive.... who did you send into the vents in the beginning of the assault? Cuz Mordin does not cut it he dies lol. Has anyone saved Kelly Chambers?

Kelly? Despite her traumatic state, she's alright.
I sent Mordin into the vents, and had Garrus as my other Squad leader. Then I used Jack as my barrier against the swarm. Just Mordin died.
Everyone...... till I decided to go to gamefaqs to see how this really goes down. I almost saved myself a bunch of time by going with Garrus again for the second part, but I fucked up by trying almost every possible choice (Even the shit that made no sense). I tend to make things more complicated then they actually are.
I've read a lot on how to keep your team alive. No one seems to have a concrete answer. Personally I think it is just randomly generated on your choices throughout the whole game. I have looked at tons of different options people have used and everyone of them is conflicting so I'm just going to keep trying by playing through again
Mordin, I was pissed, I really liked that dude :( Replaying so I can get a better ending, though I'm interested in see how the game continues with Shepard death in ME3.
I lost Legion and Thane.
I have a save right before the suicide mission though and I'll probably go back and do it over again so that everyone can live. Only because I want everyone alive for my main character, for my other characters I will experiment with different actions and see who lives and who dies.
Everyone survived including entire crew in my game. Everyone loyal too.
Mini how-to:
Just make sure to have every squad member and do their loyalty mission before going to the broken Reaper for the Reaper IFF (excluding Legion of course). After you get the IFF and Legion do his loyalty mission and you will get the Collector abduction part with Joker. You should have every squad member loyal and upgraded at this point and can go right for the Suicide Mission after and save the entire crew.

You just have to talk to every crew member after each mission.
nobody died for me, i just made obvious choices as to who to send where, Tali in the vent, Garrus leads both squads, Samara for a barrier, and Grunt goes back with the crew, everyone full loyalty all ship and character specific upgrades
There's a definite way. You have to have loyalty for every crew member (barring Zaeed), and you have to pick specialists. Tali and Legion are pure tech, nothing else, so they won't die in the vents. As for the group leaders, I dunno who will die for sure, but I know Garrus and Miranda can both lead, and they didn't die for me.
Also for the escort, pick someone good with combat, Grunt or Thane's good. I went with Thane.
Saving the ship's crew? Easy. When they get stolened by the baddies go straight to the relay, no side missions, nothing, just go right there, or you will be too late.
Stupid Jack died in my game because I sided with miranda in one little argument. Sucks ass, but whatever. I wonder if she's on my crew for the final boss fight she won't die... must try that soon.

I think I would have been disappointed if everyone had survived, adds to the drama having at least one character lose their life attempting to save humanity.
This is my major problem with this game. There is absolutely no real reason to upgrade your ship throughout the game. I spent my resources upgrading my weapons and armor which had actual benefits in-game.
Then I enter Omega 4 and Jack, Tali, and Thane die right off the bat. The matter-of-fact tone in which Miranda told me half my squad died and how easily Shepherd took the news was strange as well. I had seen more emotion from these characters in side-quests involving non-squad members.
Then I have to divide my squad and assign positions. Looking at the "correct" way to do this I followed it pretty much to the tee. Regardless Legion gets blasted in the face opening the door and Grunt gets carried off by space butterflies (which I thought was kind of funny that they picked the largest character for that fate).
So yeah all that time spent building up my squad and doing the loyalty quests and half my squad bites it cause I didn't do the ship enhancements which are only really used for that single segment anyway. Otherwise the ending twist and interaction with the Illusive Man were pretty top notch.
I posted this elsewhere, but I lost half of my team:
-Tali died, after she made it through the vents. We were trying to get the door closed, and in the process she took a bullet to the head. Loyal, but pretty sure that because Miranda wasn't loyal, she didn't offer necessary suppressive fire. More on her later.
-Jack, who was my second party member on the part with the biotic shield, which was provided by Miranda, was overtaken by the storm. Again, pretty sure this is because Miranda wasn't loyal and so instead of making sure Jack made it safely by (I mean, why would she? They hate each other), just ran along ahead and saved her own ass.
-Grunt died as the leader of the second team in that mission. Really, that's my bad; he wasn't prepared for it. Awesome death scene though.
-Zaeed died escorting the remain crew members back to the ship. He wasn't loyal though, so knew that was a strong possibility. Crew survived, including Kelly.
-Finally, Miranda was the only one that died in the final defensive group as Thane, Garrus and Shepard finished the mission. In retrospect, she would have preferred that anyway, what with how I screwed over the Illusive Man. And she was more or less responsible for two deaths in our team already, so didn't feel too bad about that.
So yeah. Some poor party decisions at the end proved pretty fatal.

Hmm that's interesting. Either way sending a full-tech member is key. Mordin's half combat isn't he?
Perhaps it has something to do with her conflict with legion. I didn't take a side, I made them be peaceful. Also in something I read you gotta have your paragon, or renegade at like 75% or higher, but who knows. I had full paragon by that time.
Everyone survived. And I mean EVERYONE. All fully loyal, with ALL ship upgrades. I saw them and figured they may come in handy. They did. I sent legion to the vents, he survived. Garrus leader. Samara shield, Garrus leader, Jacob escort, who got everyone back. Seeing how many people died in everyone elses shocks me, because I didn't read a faq, I just got everyone loyal and made logical enough choices. Maybe I just got REALLY lucky. Even kayle-IMEANKELLY survived.
For me it was Legion and Zaeed that died. Kelly died too, I didn't think you could save her. Used Legion in the pipes used Zaeed to take Doctor Chalkwas back to the ship. Also my least two favourite characters. Used the Justicar for team leader the first time and then used her for the barrier which was all good. Used Garrus for team leader the second time, looked like he got shot, but it was all good. Then finished up with Tali and Garrus by my side, kept it old school throughout most of the game. Went for Tali as a love interest.
However all of these possibilities for death leads to a bigger issue in Mass Effect 3, which is none of these characters can be big players in it. The only reason Tali and Garrus could be on your team in Mass Effect 2 is that they couldn't die in the first, so expect all new crew with small bit roles from the ME2 crew in ME3.
I got Kelly to survive. You just have to do the Suicide Mission right after the crew gets kidnapped. If you land on a planet or do any side missions, you're screwed.
Also, its seems the vents play on two factors; who was leading the support team, and who was in the vents. It seems you need either Tali or Legion for that part, but you also need a good squad leader or they won't get the proper cover they need to get the doors closed. Miranda or Garrus should suffice.
I'm going to go ahead and type in here what I BELIEVE will make sure that everybody makes it through the suicide mission:
- Upgrade the ship. There are three specific upgrades you need for it, all of which you need to ask various squad members about.
- Upgrade your squad. It couldn't hurt to buy various squad damage and defense upgrades, but also many squad members have SPECIFIC weapons just for them. They need these things, so get them for them.
- You squad needs to be loyal, especially if you're going to assign them specific jobs in the end-game. If they aren't, somehow, then don't give them assignments. They still might die, but at least they won't take others down with them.
-Make intelligent choices. Don't ask somebody to do something they're not a specialist in. There is ONE job in there that anybody seems to be able to complete if loyal and upgraded, but the rest need a few specific people to pull off. Just pay attention to each character's skills and specialities, and you'll be fine.
Soon as I got that IFF and tried to carry on with my last loyalty mission (Samara's), I had the Collectors come knockin on my door while I was on-route to Omega, leaving me without my loyal cruel and a very dire decision to be had... Save the crew NOW, or resume with the last loyalty mission.
I chose the crew.
In doing so, Legion was killed after the biotic bubble event (I was heartbroken) and Samara as well (didn't care so much...).
So, a little advice if you wanna save EVERYONE... Take care of all the loyalty missions and anything else in the Mass Effect universe before you take on the IFF mission.
Also, has anyone one had EVERYONE including Shepard die?

When Legion was trying to close the door, Garrus told everyone to line up and let no-one get in, which let Legion keep working and shut the door.
Edit: Everyone lived on my playthrough.
If everyone is loyal and upgraded, it just depends on who you set as the specialists.

I'm in the same boat. Everyone was loyal and Tali went to the vents and died. I had Zaeed leading the other group, which I assumed was the bad decision that lead to Tali dying, but I've read about other people making the same choices and having her live. I assume that it was due to a lack of full upgrading.
The one I really don't understand is Thane. He was my love interest, and with me during the second phase, with Jacob as the biotic shield. He got taken down by the swarm.
Nobody, after I reloaded the save to fix it.

Thane escorting crew back
Tali and Jack on my Team
Garrus again commanding the fire team
Tali and Jack on my team again
Everyone else as backup.
Everyone survived.
