No one died in mine.  Now I can just focus on beating it on Insanity now and getting random power achievements.

Most of the crew including Kelly, Legion, Grunt, Jack, Thane, and Samara I think.

I had Tali and Zaeed die on me. I sent Tali into the vents and she was shot while closing the door. I made Zaeed the captain of the distraction team while Samara used her biotics to get me through the swarm. Zaeed died after we opened the door for his team.

I managed to save everyone.

Nobody, even Kelly, the Cook and the Engineers made it through ;)

Kelly, which annoyed me a lot.  I had no idea that "postponing the mission" would cause anything to happen, or that you'd be able to continue after the end, so I was sure to finish everything up before going after the collectors.

Nobody, I'm too much of a perfectionist and a whore for doing everything possible in RPGs.
As soon as IFF mission was available it was made pretty clear by the game that shit will go DOWN after you do it, so i did everything possible beforehand.
didn't even know your ship crew could die or even be saved at all, but since I had nothing to do except Legion loyalty all of them lived :P
But yeah the choices for who to assign for what were incredibly obvious, almost painfully obvious, as long as you listen and read teammate descriptions (I felt those were unnecessary even) Sadly this also removed most of the tension for me.

That doesnt mean i expect party members to die on the last mission and I couldnt know for sure that it is the last mission. Whatever man, i'm just trying to communicate that it being in a thread title is pretty fucked. Lesson learned, i wont hang around the forums for a while. "

No one died. I feel pretty good about that.

Jack, Thane and Legion died while flying towards the base and Zaeed and Jacob got killed during the mission. It feels like I had a lot more casualties than most people, but the only one I would have liked to survive was Thane. Luckily I didn't care to much about the other ones.

No one, luckily. Pretty much did all of their loyalty missions, talked to them alot and chose the right person for the jobs on the suicide mission to make sure everyone lived (even the crew). And SAVE SAVE SAVE, I was up to 120 on my last save. When everyone ended up getting on the ship before me (and since I knew from media there was a chance Shepard would die), I was prepared for the worst, but he made it :)

Jack. Everyone else survived thankfully.

 During my Insanity play through I lost the following crew members.
 
- legion died in engineering on the Normandy
- tali got shoot in the face after the vent
- grunt died while causing the distraction
- thane was eat by the little Collectors bugs
- jack died while trying to hold of the Collectors at the very end of the game

Mordin the first time however I went straight back in and made it so none of them died.

You just have to make everyone loyal and pick the right people for specific tasks. i.e.  legion for the vents and samara for the barrier 

You can use Tali for the vents to, she lived just fine for me.

I got the impression that the people you talk to the least die. While Mordin was a cool character, i didn't find him useful.

 Only Mordin and Zaeed died. I was particularly pissed about Mordin. He was my favorite of the new characters. What's more is that the game didn't even show him die. After I killed the last boss and everyone was leaving, they just showed him lying dead on the ground. I was like "how the fuck did that happen?!". Zaeed died leading the second team through the door, if I recall correctly.
 
I had everyone at full loyalty. I sent Tali through the vents, and she survived. Had Jack escort us with the barrier, she also survived. I had Garrus lead one of the teams, as well, and he made it. All of the crew survived. I kept Grunt and Thane in my party through the whole mission and they lived. I don't know if this makes a difference, but I did not ever acquire Legion. Hell, I didn't even KNOW about him until I read this thread. Had to go look up who he was, haha.

Grunt went down. I barely used him, but I'd fully talked to him. Even though I barely used it him, it still had resonance. Great game. Great ending.

Jacob and Thane, just as I hoped. Boring dudes … 

Also, Kelly died. I am really bummed about that, the ship is extremely depressing now. I didn't even know she can survive at all.

I saved the whole squad, but how do you save Kelly?

Right, I saved literally everyone I can think of except some woman named Iliana (or something). I put Tali through the vents, Garrus as the 2nd squad leader both times, Jack served as the biotic shield and Mordin was the escort (good idea if you want to ensure he doesn't die). The only problem is: as you come across the crew in the pods, that one random girl melts. I manage to rescue the rest (Kelly, the Dr. etc). Just wondered if this always happens or if I didn't, in fact, get the best ending.

Everyone lived :)  I was so relieved

Lillith (the girl in the pod) dies no matter what.

My only casualty was Thane. It was so depressing, because moments before starting the suicide mission I was having a conversation with him regarding his family.

I made the obvious choices and everyone came out quite alive. 

zaeed and jak cuz they werent loyal so i brought them with me, i thought that would keep them alive.  I was dead wrong so i loaded back up and did it again and everyone made it.
 
I'm gonna start my next play though--  i was a paragon the first time--  and this time i'm gonna be a dick!

Just finish my second play though and 3 people died. Tali, Jack, and Thane. Kind of did it on purpose though to see what happens.

I was surprised when Jack, Legion, and Thane died on the ship before the mission even started.  I sent Jacob through the vents and he died while trying to shut the door.  Even though 4 people died on my mission, I'm happy with the way things turned out.

No one. Hell, I even saved my whole crew, too. Although there was this one second where Jacob walked past a door all holding his stomach looking like he was gonna die, but he was alright in the end.

i had everyone's loyalty :/ i guess its something not so obvious that i did.

I kept everyone alive.  Everyone was loyal going in.  Sent Tali in the pipe, Miranda as the second squad leader.  Samara as the biotic, with Miranda leading the second squad again, and Jacob leading the crew back to the ship.  Jack and Mordin against the final boss, though that probably doesn't matter at that point.

Picked Samara as the other squad leader which let Legion die so I replayed it and used Garrus. 
 
I'm bummed that they didn't have an 80's sci-fi flick action/adventure rock song at the credits like part 1.
