Let it be known that starting on Very Hard is not a good idea. This isn't like Dishonored or certain other modern games where the words mean nothing, when this game says Very Hard it means very hard.
I'm thinking about doing that as well, but I kind of want some progression, as the game is very short. I think I'm just good start and medium then replay each one progressively harder. I like slowly progressing in skill much more because you can savour it and relish in how badass you're becoming, instead of learning it all at once.
That's a pretty damn cool, in-joke. I'm with @xyzygy though, if you can put the normal campaign away in one sitting I'm going to work my way up the difficulty ladder, get some more time out of it.

if you are familiar with the genre, I strongly suggest starting from Hard. It will force you to learn almost every mechanic properly, bar maybe cancels, to get through the game.
