Just recently started playing, and I noticed that the top and bottom portions of the screen are being cut off of my screen. For example, the top portion of the health and experience bars are not visible on screen, and during the tutorial the text that pops up on the bottom of the screen was also cut off.

Anybody else have this issue or any idea how to fix it?

I don't own Nuclear Throne for PS4 specifically so this is just a general guess. In the XMB under Settings there is an option for screen adjustment where you line up small white lines on each corner of the screen so there is no overscan loss. If your TV/Monitor has a fit or size option on the remote you can look into that as well to make sure it is correct or to your liking as well. If you chage that setting you may need to go into the PS4 screen settings and adjust the overscan again.

I have the same issue on PS4. I changed the display output area and made it smaller than my TV. It still cuts off.

I love this game but man, some of the basic menu stuff like this is really rough.

See if you have your TVs set to 16:9 (on the actual tv settings , not the ps4) under picture size, in that same menu check to see if you have another setting called "just scan", or "auto", switch to one of these and re-adjust the display output area on the ps4, this should solve it.

I had a TV without these settings once, so I had get into the service menu with a code I found on the Internet, take into consideration that doing this with some TVs will void warranty.
