If you been reading the manga then does that mean you want to start at where you currently are? Do you want to start at where it gets "good". More recent, more older?
The issue is that the anime is half episodes using the manga and half anime only episodes. The consensus is that the anime only episodes are totally skipable because they are not the best and have nothing to do with the plot. I personally like a decent amount of them and are only recently getting tire of them (or the show in general.) Another big problem is that the story and characters are the biggest reason to watch this for me, so it's hard to say start somewhere in the middle. It's not like Dr. Who unfortunately. The characters never change.
If you want to start off with a clean slate, I say start at the beginning and watch only the manga episodes (I'm assuming you're a sane person and won't spend his entire summer break between junior and senior year of highschool watching 400-500 episodes of detective Conan.) After so many episodes you will not want to watch the show because of the detective stuff but only watch it for the characters, so it will be find to watch mostly the manga episodes. If you want to start at where you currently are at in the manga, then go the the super helpful Dective Conan wiki and found where you left off there.
