I haven't played Eternal Darkness since it came out, but I bet it holds up. It's mainly an adventure game. The combat is there, but that's not the focus.
I always forget about the insanity mechanic because if it weren't there, the game would still be good. I remember the insanity stuff tricking me in fun ways and getting me to chuckle a few times. But it was the story, atmosphere, music and gameplay variety that were what made Eternal Darkness memorable for me.
