There is mention of it in previous games in the series, but the drug itself is first introduced to the player in The Elder Scrolls III: Morrowind. Once moon sugar - an indigenous spice to the Khajiit province of Elsweyr - is properly cultivated, this illegal narcotic is produced. While Skooma can be consumed in its raw form, in The Elder Scrolls III: Morrowind it is more commonly smoked via Skooma pipes. The drug is also found in the next installment of the series, The Elder Scrolls IV: Oblivion.
