The KS-23 is a Russian made shotgun, developed during the 1970s. While it is a shotgun, it is officially designated as a carbine by the Russian military due to its usage of a rifled barrel. The KS-23 shotgun is well known for using an extremely large round, equivalent roughly to a 6.27 gauge shotgun shell using the American system. The 'Karabin Spetsialniy', or 'Special Carbine' was designed during the 1970s to suppress prison riots. The barrel of the KS-23 was actually made from rejected barrels for anti-aircraft weapons. These barrels were rejected due to manufacturing flaws, but were deemed to be of a suitable quality to handle firing shotgun shells.

The KS-23 shotgun appears only during the single-player campaign in Call of Duty: Black Ops. It is depicted as a high-powered pump action shotgun, and is one of the main weapons of the Russian military. However, it appears to see use by the SOG as well, as it is a default starting weapon in some of the missions. The KS-23 holds the distinction of being the highest-powered shotgun yet to have appeared in the Call of Duty franchise, and is capable of 'gibbing' enemies with a single shot. In contrast to its original use for suppressing prison riots, the player steals a KS-23 shotgun during 'Vorkuta', which takes place in a Soviet labour prison riot.
