I have played about 4 to 5 hours so far, and I am actually enjoying it. It's not pretty, it's not modern, it's a throw back to an old type of shooter. That in itself will be an issue for most people and be the divide between people having a good time and people who will hate it. But I can't say I'm not having fun, I've laughed a good few times, and the arcade style shooting is a nice change.
There are some pretty great levels, most fun I've had so far involves being shrunk Duke. Killing pig cops who are hiding under styrofoam cups and using burgers as a means to avoid grills reminds me of the old micro machine games (and I bloody loved those). Also when Duke spouts his one liners in his high pitched mini voice, it's hard not to smile.
I went into it knowing it would feel dated, but that doesn't mean it's bad. It never take's itself seriously, it's rude, and it's completely unashamed of itself.
Also I've been playing on the pc, which I think is the best place to play it. Load times are minimal and it looks fine, not terrible, not great but fine, and I haven't suffered any frame rate drops.
I'm not going to recommend buying it because from what I've heard load times on consoles are too long, and there are frame rate issues. But by all means rent it, just for curiosity sake to see what on earth this game has turned out like after being in limbo.
Also Duke jumping in a mirror...priceless.
