Is there a way to keep all teammates alive through the suicide mission without them all being loyal? Jack is my only disloyal party member and she dies no matter what choices I make.

She dies when she is holding the door while I fight the final boss. Who did you take into the boss fight with you ?
It's not that hard. Take the time to do all the loyalty missions, pick the right people (Don't pick the people you like, be smart about it) and I read somewhere that its best to keep Grunt back to hold off the collectors while take the big bad, because they add all the defense points together or something like that.

Not everyone has to be loyal, but the people you pick to do things must and you need all the important ship upgrades.
Leader: Miranda/Jacob/Garrus
Biotic Specialist: Samara/Morinth/Jack
Escort: Anyone Loyal
Final Boss: Both must be loyal
You must leave back some loyal tank characters like Grunt and Zaeed when you go to fight the boss. The whole group gets a defensive rating, if it's not high enough someone dies. Non-loyal people die first, but even a loyal squishy like Mordin can die if your defensie rating isn't high enough. It's best to send a squishy (Mordin) back as the escort so they're removed from the equation.
Mordin died on me the first time, so on the second time I took him into the boss battle with me in an attempt to keep him alive. I was afraid he might die during the cutscenes after the final boss (i dont even know if anyone can die like that), but he pulled through. Then I just saved my game and voila. Got everyone. I have all loyalties/upgrades, though.
I just did the suicide mission a little while ago with no deaths. Everyone was loyal except miranda (because of the fight with jack).
They do not all need to be loyal, as far as i can tell.
I had 2 unloyal ones; and didn't asign them as anything.
use this stratagy: (just don't use unloyal ones.)
Fire team leader 2: Garrus or Miranda
Biotics: Jack or Samara
Vents: Tali or Legion.
Escort: Mordin. (Mordin must be sent back otherwise he nerly always dies.)
Awesome, I did it! I used Legion in vents, Garrus as the squad leader both times, Samara as the biotic, sent Zaeed back with the crew, and took Tali and Samara into the last boss fight. Again, Jack was my only non loyal character, but she survived holding the door and now I've got my ME2 S rank.
