If the player is in human form King Jeremiah will invade their world in the main courtyard in the Painted World of Ariamis, down the stairs to the right of the phalanx.
According to the description of his armor, King Jeremiah was one of the abominations exiled to the Painted World by the Gods. He is described as "legendary", but why he is held in such high regard, as well as the reason for his exile, remains unclear.
Defeating his phantom will net the player one Humanity and his weapon, the Notched Whip. His armor can be found a corpse on the broken bridge behind Crossbreed Priscilla.
