How to get the survey to re-appear?

I clicked a link while the GiantBomb homepage was in the midst of loading only to see a survey pop-up then disappear as the other link I had clicked on loaded. Is there any way to get the survey to re-appear or a direct link to it? I've tried other web browsers and other computers but it is not showing up on any of them.

Have you logged off then back on. Just a wild stab in the dark suggestion though.

Yup, logged out and back in, no difference. Tried Chrome, Firefox, IE11, and even tried Edge, no luck.

@d00mm4r1n3 I did it when it showed up the first time but was wondering if it was possible. I cleared my cookies for the past week (or if you don't want to do that I'm pretty sure it's the QSI intercept one but not sure) and nothing happened but a little while later when I refreshed the site it came up again.
