These were assumed names used by Choko Kawaguchi, a designer on the original Phantasy Star games. There's already a page for the correct name, with game credits. The empty pages above can be deleted.

I suppose you could add the aliases of "Choko Koizumi" and "Myau Choko" (as well as his other aliases, "Choko", "Chouko" and "Cyouko"), if you're into that insane chapter of Japanese game history when everyone was paranoid of cross-studio headhunting.
