Monika
Grey area of what would be considered a boss. Monika is disturbing in the way that she interacts with the main protagonist and YOU the player personally. She is even more disturbing when you see the things she did to the other girls by having them be killed in gruesome ways and later on deleted from the game.

Valmar Heart
This thing is just a giant disembodied beating heart with tentacles and swords embedded in it. It makes beating noises throughout the battle and the transformation sequence is rather disturbing too. The vessel begins to transform into this shapeless blob of flesh that soon consumes all of the soldiers around, hence all of the swords in its appearance.

Exbelua
When you find out that the creature you were fighting is actually the sweet little old lady from the labor camp whom you just made friends with is heart breaking. The creature itself looks rather grotesque with the disproportionate arm sizes, the loud stomping, the posture that makes it lean all the way backwards, and the faceless appearance. This is made even worse when you see another character in this condition and they look even more disturbing in my opinion. Definitely scared me a lot as a kid.

Majora
Another one that many people find disturbing that I do not find as disturbing, but actually pretty cool. Regardless, those soul piecing eyes, tentacles, and weird psychedelic movements really do put it up there for me.

Mega Fatty
A gigantic, naked, obese creature who attacks by farting and spewing excrement all over is just utterly gross.

Giygas
While many people find Giygas to be disturbing, I personally do not. I find the concept behind his battle in Earthbound to be very cool actually, but I do understand why some people may find him disturbing. That being said, the concept of a being becoming so powerful that their body and mind are consumed by madness to the point that they become the abstract concept of evil does seem rather disturbing. That, and I guess his association with being a fetus and all despite those being mere hypotheses and not actually the case here unfortunately.
