The main reason we never got the Japanese SMB2 isn't the difficulty (although it may have played a role) but that the game was just old and looked bland. The Japanese SMB2 was an '86 game, and by the time Nintendo started thinking about releasing a sequel for SMB in the USA it looked really out of date and too similar to the original.
