I'm not sure they're going to be adding any fisting gameplay to Saints Row... but... I guess you never know? If they did, I suppose they would have to include this as an in game item.

oh yep no gameplay yep just keep saying shit how does the trailer not having gameplay sum up saints row perfectly? it had more mini games and more fun things to actually do then GTA 4

This and Saints Row: Drive-By are some of my most anticipated games of the year (along with MGS3 3D and Professor Layton Vs. Phoenix Wright).
