Largest number of souls you have lost?
Lost about 25,000 souls once during my first playthrough Sen's Fortress. I haven't lost any souls yet on my current NG+, which I'm almost done with. Once you know the layouts, it's much easier to avoid falling to your death, which was a common cause of death for me during my first playthrough.
7000. Was just beginning to venture into Blight Town, got killed once, then on the second trip got knocked off a platform.
40k and about 15 humanity in the sewers with the curse frogs. I was invaded but managed to kill the guy right as he killed me, so I lost his souls too I guess, then got cursed as soon as I dropped back down there. Curse is instant death and not realizing that frog breath isn't poison or toxin can really mess up your day. Also, it was so easy to resist with 15 humanity, much much harder with 0.
@TheGreatGuero: Nope. But the fucker got me again. Not gonna lie, after spending 5+ hours on this ONE STAGE, the game is starting to look less appealing. Like, am I going to have to farm souls in the opening stages for the rest of the week just so I can survive the Dragon?

Max I've lost is 18k souls in Blighttown. I initially died with 18k souls by falling to my death while attempting to walk on a large branch on the platform at the top of the big wheel. I died the 2nd time with 12k souls by falling to my death while trying to go from the big wheel to the upper platform again. Now I need to retrieve those 12k.

That exact same thing happened to me. Almost the same soul count too. But then I went back. The size of that wolf's sword won't matter all that much...IN HELL
well demon's souls around 8k getting killed by golden skeleton warriors twice. i'm early in dark souls so none but i'm pretty careful that if i'm fighting in unknown areas i use up my souls when i can.
I think it was around 10k. That might change if I screw up when I go to play tomorrow. I have 15k and 3 humanity sitting out there in Sen's Fortress, and I just can't play anymore right now at the risk of breaking something.
I just did Gaping Demon a few times and was running around with 75k souls, trying to get to firelink pyro trainer and undead parish blacksmith. I didn't die but I was super fucking paranoid the whole time. If I had lost those I would have curled into a ball and cried. I had 17 humanity too.
Eventually you will get to a point in newgame+ where a boss kill is giving you 100k+ souls. Just spend them as you get them and think of it this way: The only souls you are ever really going to lose are the ones you are going to earn right back getting to where you were when you died. The only real loss to getting yourself killed twice in a row is the humanity loss not the souls. It's also for this same reason that you should never really head back to a bonfire halfway through an area and spend souls. You are just going to end up in the exact same situation when you get back to where you were but only half as prepared as you could have been had you taken the chance and died anyway.
Don't sweat losing your souls and humanity no matter how big the loss. It's all part of the game. The real gains you will get are from seeing how that boss killed you not how many souls you saved by heading back to a bonfire.
I lost 15 000 on Ornstein because I just didn't care. Trying to get to the stain at the start of the fight wasn't a good idea, so I just gave up on it.
Though, I did almost lose 30 000 souls in Lower Ador Ruins (I can't remember names... at all, maybe that is what it was called). There is an area with a spiral stairwell that just drops at the end. I decided that since I was locked in the room by a white gate that I had to drop into the pit. I died immediately. I made my way back and got my blood stain then kept looking around for something I missed. There was nothing so I had to google it. Turns out you need a ring that allows you to walk through an abyss or something. I didn't have it and was still trapped in the room.
Then I remembered the home-bound bones I got at the start of the game. Managed to get those 30 000 souls back to a bonfire because of that item.

I was so damn stupid first death was my bad. 2nd death i was pinned in the corner by cant remember the name the goat boss the first time. Frack!
I just lost 24k souls from those goddamned ghosts in New Londo Ruins. They killed me the first time, due to me not being completely ready for one of their attacks, but the second death was from the first two you encounter. One of them knocked me down, and the other attacked me as I was getting up, never even giving me a chance to recover. I feel that it was an incredibly cheap death that should not have happened. Fuck those ghosts, and fuck this game. I mean, it's a great game, but seriously....fuck this game. I was going to do so much with those souls....
I entered The Depths for the first time. Saw a summoning text and thought "what the hell, some help would be nice". And yeah, the player who joined me was really friendly. We more or less speeded through the entire place, killing bosses and such. Then we reached Gaping Dragon, and I was an idiot (who had no idea what to do, my very first visit there remember) and it managed to kill me. The other player was kicked (happens when the host dies), and my body was in Gaping Dragon's room with roughly 24,000 souls and 4 humanity. I was grinding my teeth so much, and eventually set out to try and recover it. Of course I managed to get cursed on the way there (insta-death) so I had to accept that I'd never see those souls or humanity again.
I was not pleased, especially since I now was cursed and had to grind 3,000 souls for an antidote with half of a lifebar.
20k.. I killed the Butterfly then I went to the monster that was in the lake and I died because I DIDN'T KNOW IT WILL FUCKING THROW WATER BALLS AT ME AND 1 HIT KILL ME !! Then I went back to get them but I was mad and I couldn't concentrate and the fucking golems killed me. Epic fail! So much frustaration with this game lol... that is alot of souls...
