A prequel to Dead Space, Extraction tells the story of the original Necromorph outbreak on the USG Ishimura. The game trades the original's third-person action for a hectic on-rails shooter from a first-person perspective.

Yeah, it wouldn't sell several million copies because, perhaps, it is actually a good game? I wouldn't be shocked if people just bought a game solely on its cover (damn thy compulsive buyers).
I think you have to realize what EA is trying to do here. They are selling an M rated franchise on a "family" system. It's going to be in the same section as Mario Kart, Wii Fit, and other main-stream, mom-friendly fare. EA probably wanted something that disguised just how gory and scary a game like Dead Space Extraction can be. Dad can be shopping with the kids and Mom, show his wife this cover, and still get spouse approval. If there's a dismembered arm of the front with blood flying around? Probably not. EA knows that core gamers don't care about the box-art, because ultimately it comes down to gameplay. The mainstream does care, because they know nothing more about the game than the box-art, and EA knows this.
