No one died in mine either.

Mordin back with the crew (ironically he has a 50/50 shot at dying at the door - holding the line) / Garrus leading the second fire team / Jack doing the bubble
 
Had Legion and Garrus with me in the final fight.
 
Everyone was loyal, saved the crew.

On my first try, Garrus, Grunt, Jacob, Mordin and Jack died! needless to say, I immediately redid it. Then only Jacob and jack died. I dont give a shit about them :D

I lost Zaeed when I sent him to escort the crew members back to the Normandy, and Jack died trying to hold the position in the final battle. They were the only two characters whose loyalty I couldn't win (I pissed off Jack by siding with Miranda in an argument, and I opted to save the workers in Zaeed's mission).

  Lost Kelly and half the crew.
  
Sent Tali to the vents, Garrus lead the other team both times and Samara did the forcefield thing, sent Jacob back with the remaining crew.  Garrus and Grunt helped me bring down the end game.  Everyone was loyal and I was full Paragon with a little Renegade because I had to be a little bad ass...

i had everyone loyal and only jack and the non playable crew died.
 
it was kinda poetic cause jack was my romantic interest and i didnt have any physical relationship with her til the end which ended up being not satisfying at all. and it was funny cause i had miranda running the barrier which ended up with jack dying.
 
also i refuse to believe that there is any other person fit to run my second squad other than garrus. also legion seemed like the obvious choice to hack shit for me.
 
edit: the rest of the crew died but i didnt send anyone to help them.

I think the only reason to get everyone to survive is buying and finding everysingle upgrade and get all of the to be loyale. Witch is why I did almost everything in the game and had a 38 hour playtime when I was done.

First off, what a great last section to a game.  Fantastic.  Second, I made some stupid choices and lost three members on my run through. 
 
 I had Jacob in the vents with Tali leading the squad (hey I was smashing Tali, I can let her lead!) which results in Jacob getting smoked in the face.  I had Miranda do the biotic barrier thing and had Samara in my squad (hey she has biotics and assault rifles, ummm obvious choice to be in my squad) which resulted in Samara getting carried away to death.  Then I had Thane lead the second fire team and he got smoked.  I did rescue the NPC crew, including Kelly, and they made it back to safety with Jack.  The rest of the defensive team made it fine.  Yeah, oops.  Totally awesome though.  And I was very bummed about losing Thane and Samara as they are great characters that I used quite a bit through the game.

Here's how you make everybody survive. From the ME wiki
 

• a specialist technician is required to crawl in some tube (Pick: Tali or Legion. Don't pick: Thane, Jacob, Mordin or Garrus as they will die.)
• a leader is required for the second team (Garrus/Miranda/Jacob, anyone else will get your specialist technician killed.)

• someone with biotic power to generate a shield (Pick: Samara/Morinth or Jack. Don't pick: Miranda, Thane or Jacob will cause the death of one of your squad mates.)
• another person to escort the survivors back to the ship, and you can't use this person for the final session. (Pick: any loyal teammate [Mordin is recommended since he is likely to die on other tasks]. Choosing no one results in the death of the survivors.)
• leader for the second team (Pick: Garrus, Miranda, Jacob. Don't pick: Thane, Grunt, Zaeed, Legion, Tali, Mordin or Samara.)

How the hell does Kelly and the crew die? I didn't even know that was possible. I just ran in and saved all of the crew with no problem.

No one. All party and crew members survived.

I lost Thane and Tali during the cut scenes on the way there. 
 
Losing Tali was hard to watch, so I reloaded, upgraded the ship entirely and went again. I had all my team together and they were all loyal (actually, Tali's loyalty went down, but it didn't seem to matter). 
 
Also not sure if your specialist/fireteam choices make much different, but I did try educated guesses. I used Legion to open the doors (good with tech), I used Garrus both times to lead the fireteams (good team experience), I used Samara (uber-powerful) to give us a biotic shield and used Zaeed (knows how to work alone) to escort the team back. We all lived, hell yeah!!

Te first time I played Tali died - After I acquired Legion, she was sad and my renegade/paragon points were not high to convince her and I lost her loyalty, so she died. 
Second time, no-one.

Legion, Thane :(, Miranda.
 
Kelly also died, I don't know if this is a scripted thing or not though.

If you try your absolute darndest, how many people can you lose? A lot I'm guessing.

I was deeply relieved that I managed to save everyone including the peripheral crew. I got lucky that I went straight for the relay after the Collector attack because I didn't know that had bearing. Apparently even picking one of the dialogue choices and taking too long to do the final boss fight will get someone killed.

FWIW Rowr, I avoided this game's forum for a few weeks before release, and until I'd finished the game. If you don't want something spoiled, there's measures you can take to ensure it. "

It doesn't work that way, either. I had Legion Loyal, but I didn't talk to him entirely. I have everyone complete when it comes to socialization,  except for Legion.

This.  Had Legion go into the vent thing, Garrus as my other squad leader (both times), Samara as my shield and Grunt escort the crew back to the ship.  I was suprissed i got the achievement first time XD. "

Everyone survived in mine :D. It was difficult and I thought I made a few wrong choices but I made it out with everyone.

Miranda died. Didn't show how, just as a body after I had escaped. I put Legion in the vents.
