Everyone survived my ending, even Kelly. I was able to get everyone's loyalty (even Legion's), and got to the Collector base in time to save her. I also, apparently, gave Tali a sinus infection ;)

I managed to save all my crew before hey were turned into Collector mush and processed into the human reaper thing. I also did all the loyalty missions beforehand, and made sure I picked all the right people for the correct jobs. I had Jacob lead both of my fire teams, Legion was the tech guy, Mordin was my escort, and Samara was my biotic. I had Garrus and Thane in my team initially, and then I had Garrus and Legion.
Destroyed the base in the end. I never trusted the Illusive Man from the very beginning. No way was I placing something so powerful in his hands.
I lost mordin cause it made sense to me since you have to go to his room to upgrade shit that he would be the tech expert
" @rjayb89: I did mean squad, if you had "relations" with any of the crew members I think you lost Kelly and half the crew. But if you only flirted with kelly and no one else that half of the crew gets saved. "If you go after the Collectors right after the part where they get taken away, you can save the crew. Has naught to do with romancing anyone :)
And nobody died on my watch.
I had everyone on my team loyal and they all survived, but I didn't rescue the crew. I wasted some time doing a last side mission before going after them so only the doc was still alive when I arrived there.
Legion died in mine because I didn't upgrade my ship, I thought this was very cheap. Also Garrus died when he was Jack's shield failed (Bummer seeing as he was my favourite character).
I found it way too easy to save all your crew, and I kind of wished BioWare had made it a lot harder. My friends all lost one of their crew and had experiences I will never feel with the game, since to lose crew members now I have to purposely try to kill them...
Having done it on the first try, I'll never feel heart felt rage for Jack for killing Thane, or losing Grunt or all the various other emotions that can go through for partially failing and knowing "Next time, we'll get it right."
None, it's so easy to keep them alive...with the ship upgrades you can see all that coming a mile away
Everyone I put in front of that stupid door got hit by a rocket to the face. As a result the unfeeling robot god blasted, so Legion died. Everyone else lived though :D.

"Yeah. I upgraded my ship completely as I felt that would be a necessity and, as it turns out, it was. And when it came time to choose each team member correctly for a certain job, I felt I knew them well enough to know which person was the best for it. No one died, as a result.

I planned to go through the ending without doing any of that "Oh something bad happened, I better reload" nonsense, but I accidentally chose Garrus to go through the vents and of course he met an early demise. I said "fuck that" and reloaded. I'm not going to let my boy Garrus die because I pressed a button by accident. However, on my second attempt I got through completely unscathed.
Only one. But it was by design, as I intended(from the start)to kill Jack. What an annoying, shitty JRPG character. Glad I will never see her in ME3. Much rejoicing was had as I got word that she had been eliminated.
None, and I did it without any prior knowledge of what was to come. Guess my playstyle of doing everything in the game paid off.

None and I had no idea they could die until I read online much later. I didn't do anything specific... they all had their loyalty quests complete.
Jack was caught in the explosion by the giant ball thing.
Legion exploded in the core of the ship.
Thane was impaled by a piece of debris.
I boned Kelly, and she died. Tali was cruising through the sewers and she got popped by a stray bullet as the door closed (she was loyal). Grunt was leading the second fire team and died (he was loyal). Somehow MIranda died after the final fight with the Terminator ((she was loyal but wouldn't speak to me after I sided with Jack). I saved some of the crew, but not all (the Dr. and some other folks). It's pretty empty on the ship. Weird thing is, people I didn't give a shit about survived (Zaeed, Thane, Jacob, Legion). Some were loyal, some were not. The whole thing seemed a bit random. I pretty bummed about Miranda because she was pretty badass in a fight.
You have to have your Paragod/Renegade score high enough that when the argument happens, you can use your Charm/Intimidate option. That's why you shouldn't do Jack's loyalty mission until after you've invested a couple of ranks into your Class skill (the one that gives you a huge boost to paragon/renegade)." @Brontes_Muse said:

To your other point about how to save half the Kelly, your relationship status doesn't matter, it's how quickly you get to the Collector base. I believe you can do one mission (which should be Legion's loyalty mission), and then you have to go over there, which is why you should have EVERYthing done before you get the Reaper Drive (the mission where you get Legion). The longer you take to go the Collector base, the more people die (though a couple of random colonists always die to show you what happens if you're too late)
And as for the original topic: Grunt ate it on mine. I put him in charge of my second strike team because I thought it was going to be JUST him holding the line and I wanted it to be someone tough, I didn't realize he was going to be in a command position. Even though I didn't know much about him, his death still kind of had an impact on me. It was impressive.

Garrus died my first time through since I chose the wrong person to shield the crew from the little bugs. As soon as I realize what my careless hands had wrought I restarted. I'll never let go, Garrus.
